Abstract

Eosinophilic esophagitis (EoE) is a chronic esophageal disease mediated by the immune system. It causes dysphagia, eosinophil predominant inflammation and other symptoms such as food impaction or esophageal dysfunction. The interest in the study of this pathology has increased in the last decade due to the high levels of incidence and prevalence in pediatric and adult populations. The pathology is triggered by food antigens or aeroallergens, but the influence of the environment and the genetics is also important. EoE presents a Th2 typical response in which several cytokines like IL5, IL4 and IL13 are released. These cytokines, especially IL13, participate in the deregulation of various genes in the esophageal epithelium such as CCL26, POSTN, TSLP, FLG or DSG-1. The aim of this study is to determine the differences of gene and protein expression between patients with EoE, evaluated before and after treatment with a proton pump inhibitor (PPI) called esomeprazole. For this purpose, we have evaluated the gene expression of IL5, IL13, CCL26, POSTN, FLG and PRG2 and the protein levels of eotaxin-3, periostin, filaggrin and MBP. The gene expression was evaluated using semi-quantitative real time PCR (qPCR) while the protein levels were analyzed by inmunodetection. We also evaluated the expression of periostin in the esophageal tissue using an immunohistochemistry analysis. The results show that all of the genes analyzed, except FLG are upregulated in EoE patients compared to healthy subjects. However, the PPI-responders reached similar levels of expression as the healthy controls after the effective PPI-treatment. Filaggrin seems to be the only molecule downregulated in EoE patients but the expression levels can be restored after an effective treatment. We also aimed to identify the miRNAs deregulated in EoE patients as they could be useful biomarkers of the disease. The results obtained from the next generation sequencing show that there are some differentially expressed miRNAs between the population of patients after and before the treatment with PPI. We found 7 miRNAs upregulated and 101 downregulated, including the miRNAs previously studied that are related with EoE: miR-223, miR-375 and miR-21. The role of the miRNAs in this pathology is still unclear, so further studies would be useful to determine the definitive miRNAs signature in EoE and their role as a promising biomarker of the disease.
