This is the most common cause of hospitalization amongst patients aged 65 years and over, being the third leading cause of cardiovascular mortality. Although some works have demonstrated that the use of telemedicine in the monitoring after discharge has improved the rates of readmission in patients with heart failure, there are no many works focused on older people, which represents a population with special clinical and functional characteristics. Our main hypothesis was that by telemonitoring functional status and vital signs, we can early detect heart failure exacerbation, minimising readmissions and length of hospitalisations. date_type: published date: 2015-05 publication: International Journal of Integrated Care volume: 15 pagerange: 116972-116974 id_number: 10.5334/ijic.2173 institution: ETSI_Informatica department: Lenguajes refereed: TRUE issn: 1568-4156 official_url: http://www.ijic.org/articles/abstract/10.5334/ijic.2173/ citation: Villalba Mora, Elena and Petidier-Torregrossa, Roberto and Alonso-Bouzón, Cristina and Carnicero-Carreño, José Antonio and Rodríguez Mañas, Leocadio (2015).
